Synopsis

This economic evaluation assessed whether a physiotherapist-delivered integrated psychological and exercise intervention (StressModex) is cost-effective compared with exercise alone for people with acute whiplash-associated disorders (WAD), using data from a previous randomised controlled trial. Costs and health outcomes (quality adjusted life years, neck disability index) were analysed from both health system and societal perspectives, incorporating direct healthcare costs (physiotherapist training, health service use, medication use) and broader indirect costs (work productivity loss). Compared with exercise alone, StressModex was cost-effective from health system and societal perspectives, but with considerable uncertainty for the latter. Findings provide evidence to inform clinicians, insurers and policymakers on early intervention for acute WAD.
